PAYED ON SHOP PAGE PAYMENT PLAN - An invoice will be sent once your form is submitted. A $250 down payment is required to begin. You can make payments as you go, with no minimum amount. The full balance must be paid three weeks before the training to secure your spot.

PureSculpt Beauty Agreement

Advertisement Agreement *

By utilizing PureSculpt® Beauty, I acknowledge that I am authorized to use any advertising materials required for promotions. This includes radio stations, TV commercials, trade shows, and other platforms. Additional promotions will not result in product discounts. It's essential to note that PureSculpt must not be associated with any other ice products from other companies. I also understand that I cannot engage in false advertising using PureSculpt Beauty's name and products.

MAINTAIN CONFIDENTIALITY * *

I understand I will be trained and certified as a PureSculpt Specialist for Pure Enterprises, LLC — also commercially known as PureSculpt® Beauty and hereby agrees to not disclose any of the Company's techniques or trades unless it is expressly written to do so by PureSculpt® Beauty. I understand I will not disclose curriculum, manuals or copyrighted PureSculpt® videos to anyone who is not directly working or has not been trained through PureSculpt® Beauty. Upon completion of one or all courses provided by PureSculpt® Beauty, I will not plagiarize, copy or reprint any copyrighted material listed from the website, videos, social media platforms or printed manuals. I also understand that if I do private trainings, that I will not use PureSculpt® Beauty manuals, materials, marketing, videos, seminars, trade secrets or any PureSculpt® Beauty related materials in the trainings. This will create a breach of contract and will result in loss of access to sites, accounts, listings, product purchase and can result in legal action. I will not claim or false advertise that I am a PureSculpt® Beauty Instructor. I will not claim or false advertise that I have the authority to provide certifications under PureSculpt® Beauty. When or if in breach of my contract, I understand I will lose the PureSculpt® Beauty Specialist Benefits. PureSculpt® Beauty reserves the right to deny service or take legal action if or when necessary.

Agreement to Protect Company's Intellectual Property and Confidentiality *

To ensure the protection of the company's intellectual property and confidentiality, the following commitments have been made and agreed upon: Neither the employee nor their company, employees or associates will engage in reverse engineering, probable manufacturing, or any R&D of the company's products or similar product lines. The employee agrees to maintain the confidentiality of passwords, training videos, and other materials provided during their training. These materials will not be disclosed to anyone without prior permission.

Acknowledgment of Professionalism and Compliance * *

As a professional, I acknowledge my responsibility to adhere to state regulations. I also acknowledge that while PureSculpt® provides training and certification, it does not extend licensing to individuals. It is my duty to remain informed about state licensing laws and ensure that my practice aligns with my current licensure.

UTAH PROFESSIONALS * *

I understand that in the state of Utah if you are a Master Esthetician you can apply PureSculpt® and WoodSculpt® (light pressure) for Lymphatic purposes. I understand that I cannot use deep tissue pressure while using the WoodSculpt® tools and will abide by the State’s Regulations. I understand that I require at least 200 hours of documented lymphatic drainage. I also understand that if I am a cosmetologist or esthetician, I can only do the Full Facial and the PureSculpt® Ice body.

TAXES *

Each Distributor is responsible for collecting State and/or Local Sales Taxes (when and where applicable) and paying sales taxes to the proper government authorities. Each Distributor is also responsible for paying his/her own State and/or Federal Income Tax.

MODEL AGREEMENT *

- I understand I need to have a model and massage table for any course that I will be taking.

PAYMENT AGREEMENT *

- I understand that this is just the registration form. I will make full payment on website shop. I understand that there are no refunds or exchanges once I purchase my courses.
